Our Notice of Privacy Practices (NPP) is a legally mandated document that details exactly how we safeguard, store, and utilize your child's Protected Health Information (PHI).
What it covers:
It outlines your rights regarding medical, clinical, and psychotherapy records under federal HIPAA regulations and strict North Carolina Mental Health Laws (N.C.G.S. § 122C).
Why it matters:
It explains your rights to inspect your child's clinical records, request amendments, restrict who sees their data (such as health insurance plans), and details how we securely share data with school systems, psychiatrists, and county social workers to coordinate care.
The Intake Sign-off Acknowledgement Form is a simple, legally required confirmation sheet proving that you were given access to our privacy practices.
Why we need it:
State licensing regulators (NCDHHS) and North Carolina regional Managed Care Organizations (LME/MCOs) strictly forbid us from admitting a resident without a signed acknowledgement form on file.
What your signature means:
Signing this form does not mean you are waiving your privacy rights or consenting to random disclosures. It simply confirms that we did our job by providing you with the NPP before care began. It guarantees your file is legally compliant and ready for admission day.
